F/O that petitioner's in forma pauperis status is hereby revoked. F/O that the Clerk shall not accept for filing any further paper from petitioner unless it is accompanied by the proper fee or a new motion and affidavit to proceed in forma pauperis that contains complete and specific answers to all questions, including questions 1b and 2a, regarding total income received monthly and over the last 12 months. Additional, any paper must specifically designate, on its face, the appeal in which it is to be filed. F/O the Clerk shall not accept for filing any further paper from appellant that designateds unconsolidated appeals and appellant shall, in any further filing, file a separate filing for each unconsolidated appeal.
